Current guidelines recommend oral anticoagulation for high-risk patients with atrial fibrillation, but the inherent limitations of these agents lead to substantial underuse. This state of affairs has stimulated the development of new agents with improved benefit-risk profiles, which appear to be offer a wider therapeutic margin and low potential for drug interactions, allowing more convenient dosing without anticoagulation monitoring. Pharmacists must understand the underlying condition, possible comorbid conditions, potential of currently available treatment options (along with their probable results and possible side effects) and play an active role in educating patients on their drug treatment strategies and lifestyle changes necessary to reduce the risk of stroke in atrial fibrillation.
